A newborn baby died after a senior doctor failed to see his mother face-to-face, an investigation has found. Officials found that maternity staff repeatedly missed chances to save the life of Giles Cooper-Hall, who died following a catalogue of errors in the maternity care of his mother at University Hospitals Plymouth NHS Trust. The failings emerged just weeks after the Ockenden Report found that more than 200 babies died as a result of failings at Shrewsbury and Telford Hospital NHS Trust. The latest report highlighted how similar issues at University Hospitals Plymouth NHS Trust meant staff missed multiple opportunities to save Giles. It revealed how his mother Ruth Cooper-Hall, then aged 37, was not seen face-to-face by a consultant when she went into labour in October last year, despite recommendations made in an interim Ockenden Report published nearly two years earlier. A woman who struggled with her finances and said she was having 'really rough times' has turned her life around and now runs two businesses - bringing in a salary of £50,000. Sharing her story anonymously, the 28-year-old woman from Glasgow, Scotland, said she hated her old job and she was at her wits' end, until she met her current partner and they set up two successful businesses. Together they run a marketing agency and an education company, bringing in a total of £200,000, but they only take half that as wages and split it between them, as Glasgow Live reports. Despite bringing in £50,000 a year, the woman remembers her tough past and budgets her money each month - still putting some aside for a rainy day. Network Rail (NR) was threatened with a £2m penalty after a report found 2014-15 services were "below expectations". It has now offered more than double the proposed fine to improve services and reduce delays on Thameslink, Southern and Gatwick Express services, the Office of Rail and Road (ORR) said. London Travelwatch welcomed the move. Passengers had suffered for "far too long", the travel watchdog said. More staff ORR said the money would be used to increase station staff and track workers. Incident management software would also be introduced to help resolve issues. Boris Johnson on Wednesday opened the door to increased immigration from India to the UK in exchange for a trade deal, as he prepared to meet Narendra Modi , the country's prime minister. Speaking from his plane, the Prime Minister said he hoped to strike a trade deal "by the end of the year" and that he had "always been in favour of having people come to this country". He suggested more relaxed rules for Indian immigrants could be offered by the Government in exchange for a speedy deal that allows the two countries to trade without tariffs.
