A pilot who died when his helicopter crashed into a New York City skyscraper had been flying erratically and said he did not know where he was shortly before the accident, a report has found. A preliminary report by the National Transportation Safety Board (NTSB) said Tim McCormack had asked air traffic controllers for permission to return to a heliport on East 34th Street and then changed course and altitude several times. The 58-year-old came within 500 feet of the heliport before reversing course and striking the 54-story AXA Equitable Centre on Seventh Avenue a few blocks north of Times Square. The pilot who was killed when a helicopter he was flying crash-landed on top of a New York City skyscraper has been identified. Tim McCormack went down with the aircraft that was obliterated on impact when it struck a 54-storey building in midtown Manhattan on Monday. The real estate company that used the helicopter confirmed Mr McCormack, of Clinton Corners, New York, was the pilot. Mr McCormack was also a volunteer fire squad chief. His fellow volunteers described him as an "extremely respected" member of the service as they paid tribute. The Offical East Clinton Volunteer Fire Department paid tribute to Mr McCormack in a Facebook post as they shared an image of the firefighter. A US military cadet has died and 22 other people have been injured after a deadly accident in New York. The incident happened near the West Point academy at 6.45am local time. Images showed a large vehicle completely overturned following the incident. Those hurt include 20 cadets and two soldiers. A statement from the academy said: "One cadet is reported deceased, 20 cadets and two soldiers are wounded that were involved in a Light Medium Tactical Vehicle accident that occurred at approximately 6:45 a.m. off Route 293." The wounded have been taken to hospital and the incident is under investigation by authorities.
